Job Requirements

Specific Responsibilities:

  • Provides individual and group therapy, crisis intervention and identifies and addresses the need for education and skills training.

  • Ensures orientation of new clients immediately upon admission.

  • Obtains history and presenting information as delineated in program policy.

  • Collaboratively develops and updates an individualized treatment plan with the patient that includes targeted discharge date and plan.

  • Defines all relevant community sources affecting care and treatment of the patient (e.g. attorney, family, case manager, etc.) and develops and follows contact plans and conjoint therapy plans.

  • Updates patient-approved referral sources and family members on patient progress.

  • Utilizes patient-centered therapeutic interventions and rapport-building practices approved by the organization– interventions may include motivational interviewing focusing on non-confrontational methods to enhance and develop patient insight regarding their diagnosis and their recent patterns of substance use.

  • Documents treatment interventions, internal and external contacts, treatment plan updates and all therapy sessions according to periodicity and content requirements.

  • Maintains appointed and evolving caseload of actively engaged individuals.

  • Coordinates a comprehensive discharge plan to ensure seamless community reintegration.

Minimum Qualifications

  • Master’s degree in Social Work, Mental Health, Addictions, Psychology or related field

  • LCADC or CADC 

  • At least 2 years of experience in a behavioral healthcare or substance abuse setting serving culturally diverse populations

  • Excellent verbal and written communication skills required

  • Excellent organizational skills with the ability to prioritize workload and meet deadlines

  • Must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands

  • Exceptional analytical and problem solving skills

  • Proven ability to handle multiple projects under pressure

  • Understands diverse cultures and gender specific issues and has the ability to incorporate individual needs into a culturally diverse group

  • Understands the need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts

  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources

  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint)

  • Knowledge of case management needs for dual diagnosis patients
